About

Te Awakairangi Health Network (TeAHN) is a dynamic and innovative organisation that plans, funds and provides a wide range of primary healthcare services.

Story

The Network is one of New Zealand’s newest primary care organisations, established on 1 July 2012, drawing on the strengths of four former Primary Health Organisations (PHOs) in the Hutt Valley, and the management services organisation, Kowhai Health Trust. The Network has an enrolled population of 117,200 people and includes 23 general practices operating from 25 clinic sites within the Hutt Valley.

Te Awakairangi Health Network has a vision of ‘A healthy Hutt Valley for all’.

The Network is working towards
•improving the health of the whole population, with a focus on the needs of vulnerable groups (such as Maori, Pacific, low-income people and refugees)
•improving our collective performance in relation to the Health Targets, and
•intentionally building primary care sustainability, with increased capacity and capability.

Te Awakairangi Health Network works cooperatively with its 23 general practices, community providers, the two local whanau ora collectives, and its DHB partners. We participate strongly in the Hutt Valley Alliance Leadership Team (ALT), now called the Hutt Integrated Network of Care (Hutt INC). This group focuses on integration from a whole-of-system perspective, sharing expertise across the Hutt Valley and the greater Wellington sub region, and working within a Triple Aim framework
