A Green Prescription (GRx) is a health professional’s written advice to a patient to be physically active, as part of the patient’s health management.
The Green Prescription journey involves:
A Physical Activity Advisor contacting the patient and discussing health and nutrition goals, activity options in the community, and assisting in formulating an action plan for the participant to start with.
Follow-up reviews are then undertaken for the participant every 4 weeks over the duration of the 12 week programme. At the end of this period the aim is for the participant to be independently active and to have achieved a number of health-related goals along the way.
During the 12 weeks the patient is also supported through Nutrition seminars and resources, monthly health talks and receives a monthly e-newsletter with tips and notices.
At the end of the patients 12 weeks support on the programme, their referrer will receive a discharge summary noting what changes the patient has made and how much better they are now feeling.
